Insurance Plans
Below is a partial list of our contracted insurance
plans. We are usually in negotiation with new plans and
this may not reflect all of our contracted plans. It is
important to note that we take all plans with "out of
network benefits". We also are happy to see self pay
patients, workers compensation, or arranged IME's
(Independent Medical Examinations) or liability cases.

Will Catalina Ear Nose and Throat bill my insurance
company?
We will bill your insurance company as long as we have
the correct information. We also bill secondary, and
tertiary insurances.
Why is this charge on my statement?
Please contact our billing specialist with the account
number, name of the patient, and the charge in question.
If she is not available, she will return your call on
the following business day.
When is payment due?
Payment is due immediately. Once 30 days have elapsed
from the first statement, notices on delinquent accounts
will be mailed.
Why am I receiving another statement when I have already
paid my account?
There are two reasons that this may happen. The first is
that we mailed your statement before we received your
payment. Please call our office to confirm that your
payment was received. The second is that we have
received a payment from your insurance on a different
service and we are billing you for the remaining fees.
Does Catalina Ear Nose and Throat offer payment
arrangements?
We offer payment arrangements through our financial
lender, Care Credit. Please fill out an online
application on their site.
Billing
Payment is due at the time of service. We at Catalina
Ear Nose and Throat strive to be sensitive to the
financial concerns of our customers. If you are
uninsured or under-insured and need care, contact our
billing specialists about our payment options before you
obtain treatment. These options may allow you to
establish a workable financial arrangement before
receiving care as well as keeping your account in good
standing.

Please read and understand the terms of your health
insurance plan. Your health insurance contract should
explain in detail all included benefits and patient
responsibilities. Your contract should include the
following:
The maximum lifetime benefits you should receive.
The deductibles for in and out of network services.
The standard deductibles, co-pays, and coinsurance that
you will need to pay.
Excluded services or coverage limits.
Special requirements such as prior authorizations.
